How it works

Four parts, each one you can check.

  1. Your numbers come to Pilar, and the caller ID survives.

    Your main office numbers move to Pilar, so the patient's own number reaches your team, not your office line. Desk phones stay on your phone system; lines you want left alone, like fax, stay put.

    Check it: call from your own mobile and watch your number, not ours, appear in the inbox.

  2. It answers per location, per service line, per hour.

    Hours, holidays and a transfer order for each location. A caller asks for a person and rings the right desk, then the next one, and never loops back into the AI.

    Check it: Pilar refuses to save a transfer target that points back at a front-door number.

  3. Every call lands in an inbox with one owner.

    A one-sentence reason, the facts the caller confirmed, and a person it is assigned to. Your team works it from My assignments, Unassigned and Urgent, grouped by location.

  4. Texts go out from your numbers, with receipts.

    Each text climbs a ladder: queued, sent, delivered by the carrier, clicked. If the carrier has not confirmed it in ten minutes, your team sees “No confirmation”, not a green tick.

Behavioral-health safety

Built for the calls that matter most.

A psychiatry practice cannot run on a script someone can edit on a Friday afternoon. The safety parts of Pilar are locked, reviewed and tested before they reach a patient.

Crisis guidance is a fixed recording.

The 988 and 911 guidance plays from a clinician-approved recording in English or Spanish, never from generated speech. The call is marked urgent for your team.

Proof you can ask for: the log line naming the exact clip played.

Safety wording is locked.

Settings changes cannot remove the crisis line or the recording notice. Changing them needs your clinician approver, and every change is versioned and reversible.

Sensitive teams stay private.

Psychiatry, therapy and ketamine conversations are visible only to members of that team, whatever their role. Emergency access covers one conversation, for an hour, with a reason on record.

A new version answers a test number first and runs a scripted test pack, including a deliberately broken configuration that must fail. Safety checks pass three times out of three, or it does not ship.

What practices ask before they switch.

Will patients know they are talking to an AI?

Yes. Pilar introduces itself as your practice's AI assistant and never pretends to be a person. You choose its name; a patient can ask for a person at any point, and the call goes to the team you set for that location and hour.

Do we have to change our phone system?

Your desk phones stay where they are. Your main office numbers move to Pilar so the patient's own caller ID reaches you, and Pilar rings each location's desk phones when a person is needed. Numbers you want left alone, like a fax line or a call-tracking number, stay put.

What happens when a caller is in crisis?

Crisis guidance plays from a fixed, clinician-approved recording in English or Spanish, never from generated speech, and the call is marked urgent for your team. Settings changes cannot remove it: the safety segments are locked, and changing them needs your clinician approver.

Is Pilar HIPAA ready?

Not yet, and we will not put a compliance label on the product before the paperwork backs it. Pilar handles no patient data for a practice until we have signed a Business Associate Agreement with you and every vendor that touches patient data (phone and texting carrier, voice engine, database, call recording storage and application hosting) has signed one with us, except application hosting, which does not offer one; we disclose that to you in writing before you sign. What does it cost beyond the plan?

Nothing for seats or setup. Minutes over your plan are $0.20 or $0.18 each, texts over your plan are priced on the pricing page, and each location beyond your plan is $199 a month. We do not bill overage until your usage meter is live in your dashboard, so you can see every minute before it is invoiced.

How fast can we go live?

Pilar can answer on a test number the day you set it up, so your team hears it before any patient does. Your real lines go live when three things are done: signed BAAs, carrier registration for texting, and your number port date. We show you each one as it moves.
